NEW YORK — Taylor Swift premiered the music video for Patient Zero at the 2026 MTV Video Music Awards on Sunday, bringing Dakota Johnson and Colin Farrell into a dark, thriller-style story about manipulation, gaslighting and a toxic marriage.
The release matters because it pushes one of the four tracks from Swift’s 2025 album The Life of a Showgirl back into the spotlight, this time with a visual that leans far from a standard pop rollout. Swift said the video drew on the cinematic and literary genres she loves, including gothic ghost stories and psychological thrillers.
Swift directed the visual and introduced it while receiving an award for directing music videos at the ceremony, according to Billboard. The video is now available to watch on Spotify, Apple Music and Amazon Music, but not YouTube.
Patient Zero video puts Swift in a grim role
In the clip, Johnson plays the wife of a wealthy businessman, portrayed by Farrell, who begins to suspect that her husband is manipulating her. Swift appears as his previous wife, an heiress whose fate is revealed at the end of the video.
That ending gives the video its sharpest turn. Swift’s character is shown as a ghost trying to warn Johnson’s character to run before it is too late.
The visual also plays with duplication and dread. Swift and Johnson are dressed alike, wear the same hairstyles and are shown swimming with the same strokes, while a mirror scene places them as reflections of one another.
Farrell’s character drives the central tension. In one scene, Johnson blacks out after a single drink and apologizes during the car ride home as he gaslights her, saying, “How many drinks have you had tonight?” and “It’s like you love humiliating me in front of everyone.”
Cara Delevingne also appears briefly as the “other woman,” a role Swift’s character suspects of flirting with Farrell’s character.
A first collaboration for Swift and Johnson
This is the first time Swift and Johnson have worked together, though they are friends. Yahoo reported that the two were spotted having dinner together in New York City in 2025, and Swift praised Johnson in an essay for Time’s 2026 list of 100 most influential people.
“As her friend, I can vouch for her realness, but add that she's also one of the most empathetic people I've ever known,” Swift wrote in April.
Johnson also spoke warmly about the shoot. Speaking to The Associated Press on Sept. 24, she called filming the music video “the best.”
“It's so much fun to work with your friends,” she said. “Like I love hanging out with her, so hanging out and making stuff is really fun.”
Swift capped the rollout on social media with a post saying, “The Patient Zero video is finally out in the world!” She added that working with @chivexp felt “something out of a dream sequence,” and thanked @dakotajohnson and Colin for trusting her with their time, taste and talent.
The clip starts and ends at a grave. That final image lands hard.
