Japan Teenager Dies From Fentanyl Patch, Kyoto Report Says

A Japan teenager dies from fentanyl poisoning after swallowing pieces of a transdermal patch, according to a case report by forensic specialists at Kyoto University published in October 2026 in the journal Legal Medicine. The patch is a medical product normally applied to the skin to treat severe pain.

The death happened at the teenager's home. The researchers say the young person had a history of depression.

The paper is titled "Fatal oral ingestion of a sectioned transdermal fentanyl patch in an opioid-naïve youth with gender incongruence: a forensic case report from Japan." It reconstructs how the poisoning unfolded.

The teenager had no history of chronic pain and had never been prescribed opioids. Fentanyl is a powerful opioid, and the patch was not meant for this person.

What the autopsy found

During the autopsy, specialists found fragments of a fentanyl patch in the stomach contents. Toxicology tests then identified fentanyl and norfentanyl, its main metabolite, in the biological samples.

They also detected pregabalin, a drug used to treat certain kinds of nerve pain and other conditions. The researchers said its depressant effect on the central nervous system may have contributed to the death.

The report also notes exposure to substances associated with benzodiazepines, a class of sedatives. Combining the anatomical and toxicology findings, the investigators concluded the death was linked to misuse of the patch by swallowing it.

The authors caution that drug concentrations measured after death are hard to interpret. Drugs can shift within the body after death, which changes how they are distributed.

Why the case matters in Japan

Japan has historically had a relatively low rate of opioid use disorders compared with several other countries. The researchers warn, however, that misuse and diversion of prescription medicines is a risk, particularly for adolescents and young adults.

Fentanyl has legitimate medical uses. The danger arises when such potent drugs are taken outside medical instructions or reach people who were never prescribed them.

The case shows the risk is not confined to the illicit drug market, the report says. It can also come from medicines made for legitimate treatment.

The report does not conclude that depression caused the substance misuse. Nor does it support blaming the death solely on a mental health problem.

Prevention, the authors say, should include education for adolescents, family support and access to psychological and psychiatric care. They also call for appropriate controls on highly potent medications.
